Patients and Doctors

EMERGENCY ADMISSION

An Emergency Holding Room is available for patients with stable condition after Emergency Room Procedures and is waiting for their attending physician or room. Our Admitting Staff will approach the patient and/or relative(s) to facilitate admission.

NON-EMERGENCY ADMISSION

The patient must secure an admitting order or referral from the attending physician during his appointment. The patient can reserve a room through the Admitting Section prior to the scheduled admission date.

ROOMS

ROOM RESERVATION

Room reservations are processed by the Admitting Section and applicable only to special medical cases.

ROOM TRANSFER

You may transfer to your room of choice as soon as it becomes available. Arrangement may be made through the Admitting Section.

ADMISSION GUIDE FOR PATIENTS

  1. Admitting Staff interviews the patient and/or relative.
  2. Patient/Relative presents 2 valid IDs.
  3. Patient/Relative signs the Consent for Admission Form.
  4. Admitting Staff will confirm if patient will use HMO, health insurance or Corporate Credit Line.
  5. Patient must secure Philhealth Claim Form 1 and 2, and Claim Signature Form if he is a Philhealth member.

ADMISSION KIT

Admission Kit is available in selected rooms. If kit is not provided, you may request for a kit for a minimal fee. The Admitting Section shall provide the admission kit containing the welcome brochure, clinical nutrition leaflets, patients & family bills of rights and responsibility, and may go home reminders.

ADMISSION KIT

The discharge process is to initiate the May Go Home (MGH) order through the attending physician. This should be executed a day prior the discharge date.

Early preparation requirements should be done which include the following.

  1. Final diagnosis, performed procedure, and operative report.
  2. Home medication and other pertinent instructions.
  3. Follow-up check-up date and other requirements.
  4. Professional fee

PAYMENT SCHEDULE

  • Our Cashier Department is open 24/7.
  • The hospital accepts cash, credit cards, online payment, bank to bank transfer, and manager/cashier checks.
  • Personal Checks will not be honored.
  • Debit and Credit Card payment will only be accepted for hospital bill.
  • Professional Fees are billed separately and shall be paid through cash.

PHILHEALTH REQUIREMENTS

Philhealth requirements must be submitted to the Philhealth Section prior to discharge in order to avail the discounts.

HOSPITAL BILL

A full settlement of your hospital bill including the doctor’s professional fee is required before being allowed to be discharged. Settle your hospital bills only with our cashier. You may pay in cash or through major credit cards (Visa and Mastercard).

If you require clarification or assistance regarding your bills, please contact our Business Office.

DISCHARGE AND CHECK-OUT TIME

The hospital check-out time is at 3:00PM. There is an additional charge if you fail to vacate the room beyond the check-out time.

Kindly settle your bills as early as possible to avoid additional charges.

Patient Experience

1. Patient/Relative may schedule admission through Admitting Department via phone call or walk-in at (043) 773-6800 local 2231 / 0917 809 4386 from 8:00 AM – 6:00 PM.

2. Necessary information will be collected by our Admitting Department to fill-up the Room Queuing Form that is required for the reservation of the patient.

  1. All pre-works will be rendered
  • MD Orders
  • RT-PCR of Patient and Relative
  • Self-pay/Philhealth
  • HMO LOA and Pre-approval (if applicable)
  • (2) Valid IDs

4. Admitting Staff-on-Duty will interview the patient/relative and will discuss the guidelines and consent of the admission through phone call prior to the admission.

5. Admitting Staff-on-Duty will reserve the room for the patient’s admission.

6. On the day of the admission, patient/relative will be asked to sign the discussed consent prior the admission.

7. The patient/relative will be accompanied to their reserved room and will be endorsed to the Nurse-on-Duty.

Planned Admission
